CMS announces next steps for $50B rural healthcare funding program

Rural health in the United States is about to receive boost of support. All 50 states applied for the $50 billion Rural Health Transformation Program, a federal initiative created under the One Big Beautiful Bill Act and administered by the Centers for Medicare & Medicaid Services (CMS). The program represents a major effort to shore up rural health care systems nationwide, which are facing threats from $511 billion in Medicaid cuts and continued hospital closures. 

The five-year program, beginning in fiscal 2026 through fiscal 2030, allocates $10 billion annually to help states expand access, improve quality, and promote sustainable, innovative rural health care. CMS estimates the investment will substantially boost funding for rural hospitals beyond the estimated $19 billion in Medicaid spending in 2024. 

Funding will be distributed in two phases. Half of the $50 billion, or $25 billion, will be shared equally among all states with approved plans. The remaining $25 billion will be awarded at the discretion of CMS, based on factors such as rural population, provider distribution, and facility needs. Even if all 50 states’ submissions are approved, funding will likely vary. Some states could receive around $500 million, while others might receive well over $1 billion. States facing larger cuts could receive more in facility-based allocations. 

Federal officials described the program as the largest rural health investment in U.S. history. The $50 billion fund would offset about 37% of projected Medicaid losses in rural areas, and CMS will have broad discretion over how the money is distributed. 

CMS will review applications for completeness and merit, announce approved awardees by Dec. 31, and begin distributing funds. The program’s goals are to improve preventive care, ensure sustainable access, strengthen the workforce, test new care models, and expand health technologies.
